Aloha Kapolei Middle School 6th and 7th Grade Families,

2019-2020 School Year New Student Registration

Kapolei Middle School will begin accepting new student registration for school year 2019-2020 starting March 1, 2019.  Registration will be accepted 8:00 am - 3:00 pm. daily except for weekends and holidays.

Please bring proof of residence with you at time of registration.  See below for a list documents to prove physical residence in Kapolei. 
  • Birth certificate or current passport.
  • Legal guardianship/custody documents if parents/guardians are not listed on the birth certificate.
  • Two pieces (2) of documentation for proof of residency in the Kapolei Middle School district (i.e. current mortgage statement or rental agreement and one (1) utility bill.

will be accepted at Kapolei Middle School between January 2, 2020 and March 1, 2020.  Forms are available at applicants home school's front office.  Attach a self-addressed stamped envelope with the GE forms.  Incomplete forms will not be accepted and returned to applicants.

Enrollment Procedures:


All 6th, 7th and 8th grade students who reside within the Kapolei school district either with their parents or legal guardians are eligible to attend Kapolei Middle School.  Students who transfer from other schools, private or public, must bring a release form from their previous school.

New Student Enrollment

New students (excluding feeder school or current KMS students) who are planning to enroll at Kapolei Middle School must provide the following documentation before the enrollment process can begin:

  • Release/withdrawal papers from previous school (i.e. Certificate of Release), final report card/withdrawal grades, last date of attendance and copy of the current IEP/504 (if applicable).
  • Current Tuberculosis (TB) clearance with negative PPD results in "mm" reading (to also include date given, date read  and signature of the physician) and the results of complete physical examination both to have been done within the past twelve (12) months.  A completed Health Record (Form 14) and 7th grade Student Immunization Record (yellow VAX card) is required for all incoming 7th graders, mainland and foreign students.
  • Birth certificate or current passport.
  • Legal guardianship/custody documents if parents/guardians are not listed on the birth certificate.
  • Two pieces (2) of documentation for proof of residency in the Kapolei Middle School district (i.e. current mortgage statement or rental agreement and one (1) utility bill.

School personnel will investigate any suspicious addresses to verify residency.

Release Procedures:

If your child and/or family moves out of our school district, to the mainland, a foreign country, another island within the state or if your child will not be returning to Kapolei Middle School for the upcoming school year, please inform the Registration Office at least one (1) week prior to your child's last day of school.  The parent or legal guardian of the child will be required to come in and sign an official request for release so the official withdrawal process may begin.  We will not start the release process without a parent or legal guardians signature of consent.  Forms will not be mailed or faxed to you.  You will be asked to provide a forwarding home address, phone numbers and the name of the school your child will be transferring to (if possible).

Every effort is made to ensure that a completed Release Packet (official release and copies of school records) will be available for pick-up after school on your last day.  In the meantime, you and your child will be responsible for taking care of all outstanding student obligations before leaving Kapolei Middle School (i.e. returning all textbooks to teachers, returning borrowed books to the Library, clearing outstanding balances on meal accounts, etc.)

Upon picking up your child's Release Packet form KMS, hand-deliver said packet as soon as possible to your child's new school in order to enroll your child.  When KMS receives an official Notice of Enrollment from the new school, the remainder of your child's school records will be forwarded.  Do not open the Release Packet.  The new school will not accept the packet if the seal has been broken.
